Connections Case Management, LLC is committed to providing appropriate services for individuals of all ages and abilities in all of Indiana’s 92 counties. We provide services to those who participate in Indiana’s Home and Community Based Family Support Waiver (FSW), Community Integration and Habilitation Waiver (CIH), Health and Wellness Waiver (HWW), and the Traumatic Brain Injury Waiver (TBI).
